Operating rooms (ORs) have long been among the largest cost centers in hospitals, representing a significant portion of healthcare spending. While previous research has extensively explored the use of optimization techniques for OR planning and scheduling to reduce costs, less attention has been given to behavioral regulation as a means of managing OR costs. This study addresses the gap by investigating the impact of monitoring—a common behavioral regulation tool—on surgical supply costs in a non-standardized, multitasking setting, where controlling supply costs is an auxiliary task while ensuring surgical outcomes remains the top priority. Using surgery-level data from a representative hospital in China, we investigate the effectiveness of a monitoring system through a regression discontinuity design. Our findings provide robust evidence that adopting the monitoring system reduces supply costs per surgery, without compromising surgical outcomes. In addition, the cost-reduction effect is more pronounced among surgeons with greater work experience and those who participate in online healthcare communities, suggesting that the effectiveness of monitoring is shaped by a surgeon’s ability and preference to manage multiple tasks. Furthermore, we identify surgeons’ strategic responses to monitoring, evidenced by their greater reduction of supply usage in surgeries for insured versus uninsured patients. These results highlight both the potential benefits and limitations of using monitoring to reduce unnecessary OR costs, offering valuable managerial insights for hospital operators and policymakers.
